What's the actual URL limit for one sitemap file?

50,000 URLs and 50MB uncompressed, both from the sitemap protocol itself, not a search-engine-specific rule. Go over either and you need a sitemap index pointing to multiple sitemap files instead.

Does this follow a sitemap index automatically?

Yes. If the URL you provide is a sitemap index, the tool counts total URLs across all its child sitemaps rather than just the index file itself.

Why does the namespace matter?

Some search engines and tools reject a sitemap without the standard xmlns declaration, even if the rest of the markup is otherwise correct.
