When do I actually need a sitemap index instead of one sitemap?

When you exceed 50,000 URLs or 50MB in a single sitemap file, both real limits from the sitemap protocol. Some sites also split by content type even under that limit for organization.

Do the sitemap URLs need to already exist?

This tool generates the index file only, it doesn't check that each listed sitemap URL is live. Make sure each one is actually published before submitting the index to Search Console.

What if I have more than 50,000 sitemaps to list?

That's an extreme edge case, a sitemap index itself is also capped at 50,000 entries under the same protocol rules.

Where do I submit the generated index file?

Submit its URL in Google Search Console's Sitemaps report and in Bing Webmaster Tools, the same way you'd submit a single sitemap.
